Health Care Home and Chronic Disease Management Programs
Don Medical Clinic was enrolled in a pilot program - Health Care Homes, from 2017 - 2021 . This was a new program with federal funding that provided an alternative funding for health care, where a bundled payment was paid to the practice for a persons health care needs , in contrast to the traditional fee for service that is Medicare.
The practice continues to utilise knowledge, tools and skills gained from this project in the practice.
